The small scale change project focuses on facilitating glucose monitoring and control during the post-surgery period. The issue of concern is that failing to monitor the glucose levels could hinder healing efforts. In addition, non-controlled glucose complicates the care process. In the short-term, the project intends to improve efforts to monitor glucose levels in the post-surgery period. In the long-term, the project intends to speed up the healing process and reduce incidences of complications. The data collection plan includes records of glucose monitoring data, speed of wound healing, and feedback from post-surgery nurses.
